Skokie Indians Baseball Forms and Stuff

Baseball runs on paperwork — lots of it!

Here's where you find forms and stuff for Skokie Indians Little League. These all need Adobe Reader for viewing or printing out. Please see the link in the left-hand column to get your free copy of Adobe Reader. Other baseball leagues may find these useful as well. They are welcome to use these forms provided due credit is given to Skokie Indians Little League.

2008 Skokie Indians Little League Baseball Registration


Click here for the 2008 Registration Form.  Please note that the dates on the Skokie Park District winter program are incorrect.  The dates on this PDF form are correct.  Registration for AAA and Major divisions players are due December 28, 2007.  Registration for Rookies, A, AA and Pony are due February 1, 2008.  AAA and Major players will need to attend our Evaluations. An Evaluation reminder flyer will be mailed to your home.

If you do not receive a Evaluation reminder flyer by January 3, 2007, please contact our Evaluation Coordinator, Jim Guthrie by e-mail.

2007 Player Availability Calendar


Click here for the 2007 player availability calendar. Please follow the instructions at the top of the form, fill it out and give it to your manager. Your manager finds this essential to iron out scheduling problems if you can't be present.

Little League Scoring Form


Click here for scoring forms. These print out on 8-1/2 x 11 inch sheets of paper and were designed especially for scoring Little League and Pony League games. For managers, coaches, players, parents, relatives, friends and anyone who wants to score a game. You use two sheets per game, one for each team. The second page is a cheat-sheet of common scoring abbreviations.

Little League Pitcher Record Form


Click here for pitcher record forms. These track number of pitches thrown and the result of every pitch and at-bat. At some point, Little League of Williamsport — the Mother Ship — will require this sort of record. We thought we'd get everyone used to it early. These print out on 8-1/2 x 11 inch sheets of paper. The first page is the form itself. The next four are instructions. The last page is a cheat-sheet of abbreviations.

Skokie Indians Game Report Form

MANAGERS AND COACHES: click here for a handy, printable form to keep track of all the game data you have to report to the league. Print out a bunch and give them to your scorer. It is not for reporting the game data and cannot be filled out directly on this website.

MANAGERS AND COACHES: are you tired of those little, bitty lineup cards that came with your scorebook? Have you run out of them? Click here for a big, two-to-a-page lineup card that actually has space for players' names.

Criminal Background Check Release Form


Click here for the criminal background check release form. For the safety of our players, we and the Skokie Park District require that every Skokie Indians manager and coach be checked for possible criminal background. The Skokie Park District and the Skokie Police Department thoroughly investigate for both state and federal convictions.

"Visit Our Website" handouts


MANAGERS AND COACHES: Click here for "visit the Skokie Indians website" handouts. Give these to your players and their parents. Print out plenty so they can give them to friends and relatives. The first page is handbills printed four to a page. The second is the full page version so you don't have to mess around with scissors.

Three-Team Baseball


Click here for instructions and a scoring form for three-team baseball. Three-team baseball is excellent for practices. It's also something to do — rather than just go home — if one or both teams don't have enough players for a regulation game.

"Three Walks and You're Out"


Click here for the rules of "Three Walks and You're Out." This is a pitcher-development game for pitching practices and also an excellent way to qualify a wannabe pitcher to pitch in an actual game.
